Do we accept insurace?

Mind Body Physiotherapy is 100% cash based, meaning you will be responsible to pay before or at the time of visit. If you have insurance, we may be considered an Out of Network provider, and if so, we can provide you with a “superbill” to submit to insurance for reimbursement. Please check with your insurance company to verify Out of Network benefits for physical therapy and ask for an estimate on how much you can get reimbursed.

What forms of payment are accepted?

Cash, debit, credit, HSA/FSA are all acceptable methods of payment.

What should I ask my insurance company prior to my visit?

• Do I require pre authorization for physical therapy services?

• Which ICD 10/CPT codes are covered/reimbursable?

• What is my Out of Network deductible and has any of it has been met?

• Do I have a copay/coinsurance for I Out of Network deductible?

• Do I have an annual visit limit for Out of Network physical therapy?

How long are evaluations?

All evaluations are 60 minutes long and a series of questions will be asked, this includes a physical evaluation/movement screen. We will also spend time talking about your goals and the duration of your plan of care.
